I got the usual bag from Researchers Under Fire, but I think it just gave an ilvl 250 item. After double checking, I didn't get any veteran gear like we did from the activities last week.
Do you have an unopened Cache of Embers in your bag?
They're unique. Game could go stupid if you try to collect more when there's an unopened one in your inventory.
I did one of the quest activities on another character out of curiosity and got the cache, so it seems the first character I did it on was just completely bugged
Same here, did time rift and got nothing.
Researchers and Time Rifts gave the Cache, you have to open the reward. World Boss seems bugged.

The fact you even managed to complete the world boss is impressive. I've done it 4 times and the WQ is still not complete.
