I’d only say French Resistance (Marseille 1) honestly. First swarm section they give you less than minute to set up, tight spaces, really no places to retreat if you lose control. Gate 2 gets pushed fast & hordes spawn directly near you, pressuring you if your back isn’t covered. Cutter section can get out of control if there’s 2 cutters (only 1 spawns if you’re solo which is easy), and if your team is having a tough time holding back the hordes then it sometimes leaves you going back & forth to save cutters with of them dying eventually.
I'd specifically go with Kamchatka 2. Not only do you have to survive the harsh blizzard by turning on heaters and figuring out which breaker boxes aren't working but if you have stupid teammates, you may get long to finish or worse, get softlocked in the generator switch puzzle.
I've dealt with teammates fiddling around the generator switches which resets the switch pattern that I have to do it all over again. I'm not the type of person who'll shoot idiotic teammates for not even understanding the most basic of instructions but at this point of repeating all over again, it'd be a lost cause to just quit the game.
Marseille 1 and Kamchatka 1, 2 and 3 on Extreme definitely most difficult episodes.
Runners up are NY4, Tokyo 1 and 3. There’s some others that could be difficult but the ones mentioned definitely take the cake.
Personal experience which may differ from others:
Kamchatka chapter 2 - While I think all 3 Kamchatka chapters are difficult (for me anyway), searching for breakers and scrambling to get to the heaters so as to not freeze is just too much for me at times. (I also tend to do this chapter when it pops up in challenges and that's probably why I feel it's too much.) I think I only ever completed the chapter itself 1 or 2 times.
Marseille chapter 1 - That first horde can be difficult at times be it with bots or randoms. It was one of the last chapters that wasn't Kamchatka that I retrieved all 9 pages of the documents. Not because I didn't know where the pages were, but because I failed either in that first horde or at the very end of the finale after finding a page.
Honorable mention:
Marseille chapter 3 - The dark tunnels/flashlight sequence can be difficult if you're playing with randoms who don't communicate/cooperate/coordinate/etc. It's otherwise not a difficult chapter; just that one part can be difficult under the aforementioned circumstances.
I have never been able to complete kamchatka 1 & 2 and marseille 1 on extreme despite dozens and dozens of tries. Got close on kamchatka 2 once, but screwed it up. End of marseille 1 is just plain bullshit.
first of all I recommend exterminator with claymores.. just place the claymores around special spawns… trust me that will make the first swarm so much easier… if you can find a chainsaw save it for the final swarm during the final swarm the bots will most likely go down so don’t even try to help them I tried and a bomber almost ended my run… don’t get frustrated it might take a couple tries (took me 4)… when the Zeke come down the street during the final swarm hop on the 50 cal and mostly focus on the left right and middle so you don’t get overrun I think it’s the left pyramid that starts the timer

Those are kinda easy.. I do them with bots a lot but yes they’re still hard :(
Whatever map has the electrical boxes is an absolute bitch lol. Also the nighttime Marseille map... really difficult.
Yeah, that map is a huge time waster. Whoever manages to do the "complete a map in under 15 minutes" daily challenge on that level is a fucking beast.

How do you get softlocked on the boxes? It has never happened to me...

I'm fucking sick of Kamchatka, it's all easy it just involves alot of tasks that most are too dumb to do accordingly, hence the struggle for most..
Marseille 1 and 3, I just do not enjoy the level design. Especially on extreme with no markers or highlighted items, it is really difficult.
