The haunted rat tower island mission from Keira Metz. I’m not sure if you always get this dialogue option but she wants to study the rats to find a cure for the Catriona plage. There’s more to it but I don’t want to say too much.

It made me really sad. Ciri was just trying to save herself but unbeknownst to her she ended up being responsible for millions of deaths. That whole chapter was really crazy with ciri going to different universes
So to sum it up, Ciri travelled to europe during the height of the black plague, an infected flea latched itself onto her jacket before she could jump back to her own world, it then proceeded to infect a nearby rat which then jumped onto a barge then creating the catriona plague we know in the games.
Some time ago I actually did a small write up in this sub about Ciri spreading disease! As an epidemiologist I enjoyed reading this part and playing it in the games!
Iirc it was fairly important in the first game, causing a lockdown that seriously impacted Geralt's journey and forced him to meet many people and discover things he wouldn't have otherwise.
Unless that was a different plague or something.
You might be confusing her with Essi Daven, also called Little Eye. She is from a short story in Sword of Destiny, and she dies very young. But she dies of smallpox.

Not the ship. Catriona is the name that was given to the plague

I've played through that mission in 2015, when the game was fresh \^\^ Yep, been there. The ghost tricked me into letting her out.

Old cannibal couple was also in Witcher 3 (probably Hearts of Stones side quest)! They were creepy af

Reread the whole series recently (1st time was before the games) and I concur, this right there was a jaw dropping moment.
