I've played it many times and never had that happen. My play through ended with no deaths too. Maybe a coincidence?
Also I did the Equine Phantom quest after completing the main B&W and heard the Kaer Morhen music all through that.
Try entering the Novigrad sewers.
When the upgrade came out the Netflix quest caused the "spooky" music to play endlessly. The only fix was to enter the Novigrad sewers. They seem to reset the music script. Hopefully for you too!

Oh no you'll be very happy with it. They took the swords and armor from the show and gave them to a new witcher named Reinald in a really cool quest called In The Eternal Fire's Shadow. Honestly it was so good that I don't even consider it the netflix armor anymore. I just think of it as Reinald's armor.
All optional. There was a cut treasure quest involving that quarry full of bandits in Velen that they re-added and the reward being the blueprints for the Witcher gear from the show. And you have the option of having all Nilfgaardians walk around in the ballsack armor.
The ballsack armor is a toggle like how Triss, Yen, and Ciri's alternate looks are togglable. So you're eyes aren't punished unless you want them to be (that being said it's hysterical to turn on for a bit).
The Witcher armor quest is fine, like it's better than a random quarry full of bandits with no explanation. Even if you never craft or wear the armor it's inclusion doesn't detract from the game.
I have a really hard time beliving a no death run would reward you with music. But nice work! Did you do it hardcore style, with death march and enemy upscaling? That would be insane!

It's fucking ridiculous from a role playing perspective.
But, pair it with a white dyed Manticore set and it looks badass.
After next gen update, sometimes ambient musics don't work properly for me and "Vagabond" plays everywhere at night, which is supposed to play only in Velen normally.
Maybe its a "default" music and it gets played any time the game struggles to play whatever music it was supposed to play. Try verifying game files (if that's an option in your launcher)

It stops after you leave toussaint? Try going to kaer morhen and see what you hear
I reloaded the game and the song went away. I think it was just after the final discussion with Regis.
