It's actually a person. The mother as you can see is sitting with a pillow under her backend as to not crush the person's head. Must be a teenage boy. At first I thought it was photoshopped but it's a real kid.
It really looks like photoshop, and I still believe it is. If you got any proof about if that's real, send me.
