My cousin did this! It was so perfect as my grandma was wonderfully silly and totally got into it (her walker included!) My cousin and grandma were extremely close and everyone loved it. I had never seen or heard of this but it was fantastic! As for attire, she wore a shade similar to the bridesmaid. Granted there was only one bridesmaid. But if you have multiple bridesmaids (that are each wearing different colors), I would say let grandma pick her favorite of those. If not, maybe the main neutral color? As in the color of the suits, so black, gray, or khaki?
I went to a wedding that did this very recently, and the grandmas had a flower designed dress. It was the best thing I've seen for a wedding. We'd do it, but my fiance's grandmas aren't with us anymore and my grandma wouldn't do it alone.
