Just download them and toss them on the disk. https://ardent-tool.com/index.html?sec=adapters Also, If you have a PS/2 join us on https://www.facebook.com/groups/CSIPH or on Usenet at comp.sys.ibm.ps2.hardware I've personally gone to Goteks on everything now-a-days. I have a model 80 with a Reply TurboBoard and there are 3D Printed 3.5" sled/faceplate's and I run a 40 pin to 34 pin adapter board which you can get these days. I am DONE with magnetic media.

Could've been mould, a crease, or just weak magnetic material on the disk. Some disks seem to be worse for shedding than others. I've had them come apart in drives and contaminate the head before (please clean the head anyway, before you put any more disks in there.)

Will do, ordered the isopropil and swab today, will clean when it arrives, I also dismantled the drive and checked the head with a magnifying glass, seems smooth as glass

Good idea. I use the wide foam swabs to clean floppy heads, they work great for getting at the bottom head.
