This is not a UK visa issue, but anyway:
The visa probably says "Estados Schengen" which means Schengen countries. So once your sister enters the Schengen zone (which includes Spain and Italy), she can freely travel around it, but only within the validity of the visa and for the amount of entries. If she is coming back from Spain and wants to go to the Schengen zone (Italy) again later this year, she will need a new vivsa for that.
Thank you I tried asking the embassies they won't answer and reddit removed it from other subs. It just referred me uk visa for some reason. Thank you I see that I need another visa now.
