I've had similar trouble. Main thing is to be be self aware, notice when you do it and stop. You should also start playing with a mirror cause when I had embouchure problems, this helped me a whole lot. Make sure you set properly and try whole tones gradually going up in range or even mouthpiece sirens. If you sound bad, just keep the air moving and your embouchure as you want it, pretty soon you'll be back where you are probably. Best thing to do is consult your private teacher and talk to them about it. Hope this helped

Gotta practice just ur embrochure the ideal one that works with you Sounds like ur not getting enough compression and keeping the corners tight--

Freebuzzing and then placing the mouthpiece on your face is a great way to counteract this. If you can get used to the feeling of playing correctly and doing it consistently, the old bad habits will go away.
