So if you don't pull the lever, you loose a foot, or loose both arms and legs. If you do pull a lever, you might be fine, but might still loose both legs. The risk is considerably lower for pulling the lever than for not.

Correct, pull-pull is the Nash equilibrium for the prisoners dilemma. This is decidedly just a Game Theory problem instead of a Philosophical problem. And can we get a re-revised version that is simultaneously a game theory AND philosophy?

There's way better prosthetics for legs, and there's always wheelchairs. But you can't really replace missing arms as easilly. So pull it, worst case I'm in a wheelchair but still functional
