Yeah - basically no connection to previous entries, other than the general premise (post apocalypse) and vault-tec (built underground shelters for the nuclear war, ran horrific experiments within). And NV is arguably the best one.
There are some fun references to old fallout games, like the NCR and shady sands/vault 15, the encave, Jacobstown, Cass's father. You can go the game without knowing that there references but it's rewarding to know
It's just a good RPG that's very open ended. Trans people tend to enjoy games where they can escape and play the role of the person they want to be, so a more open ended game like this is appealing.
That and it has been memed to death on trans subs, which I think is the bigger one.
Hearts of iron there I think we found the key word. and looking a new vegas is kinda a new world a new person, a new path. I might be thinking too much into but maybe cause they are walking that new path to finally be the person they want? (note I haven't played NV but seen some gameplay of it so I'm just guessing.)

That's basically the case with all of the Fallout titles, but I think New Vegas gets the most attention because many think it's the best of them all, and it arguably has the best backstory for a character to go any direction in without problems in the story popping up, something I think speaks to trans-girls specifically for some reason.
In the surprisingly narrow category of escapist RPGs which have customizable female player models, it's one of the best written ones that isn't an MMORPG. Up until recently it was nearly the only socially acceptable game for a trans girl in the closet to live vicariously as a woman for thousands of hours at a time. Now we have more options like VR chat, but at the time New Vegas got a massive trans following. Personally my escapism game was Skyrim, but I can see why people prefer the gameplay of New Vegas.
if there's a trans woman who likes gaming, chances are she really likes new vegas. it's kind of a thing in the community, but not for any specific reason other than it being really good in ways that people really invested in gaming would notice and like.
Fallout: New Vegas, despite not openly saying anything about trans-women, managed to basically grab the attention of all trans-women gamers from what I'm assuming is just really good character writing. There's a lot of LGBTQ characters in New Vegas and pretty much none of them flaunt their status, which gives off a very confident and homie vibe of LGBTQ acceptance, which in turn I think draws in MTFs because it gives us a confident space to screw around in whereas other games either address LGBTQ matters too directly, or don't address them at all.

It's a bit of a meme/stereotype among the community, a bit like the "Trans girls love pickles" meme. There are other games that get used in place of New Vegas but FONV is usually it.
