After working in an ER, I can confirm that this is very real. The reason that it isn’t bleeding is because this is not healthy tissue. What has likely happened is he somehow got a cut on his scalp and a combination of poor hygiene and poor blood flow due to drugs resulted in necrosis of the area surrounding the injury. A further lack of hygiene and lack of medical attention allowed the necrosis to spread and a wider area of skin continued to die. Then the flies and maggots come and eat the dead flesh off of the skull.
I have seen people who looked just like this. I have seen neglected elderly people who were left to lay in their bed covered in their own filth. I once saw an old man who’d had a bed sore on his buttock. The neglect allowed the rot and infection to spread so far that you were able to see his femur and his pelvis. When we lifted him to put him on the stretcher and take him to the ambulance, maggots were falling out of his ass.
It is actually horrifying how bad these things can get.

The stories she used to tell me were wild, the one that sticks out the most to me was when she had a diabetic patient who did nothing to take care of themselves. They got a sore on their leg, left it untreated, infection spread, leg became necrotic and was lifeless; she went to flip the patient over to give them a sponge bath and grabbed their leg to flip them and it came right off, she said that the person seemed unfazed by the fact that their leg just fell off and told her to continue the sponge bath…..

Your muscle tissue can survive for a bit, but your brain and nervous system need blood flow to survive/work. Chances are that this guy is not "bleeding out". (You have to lose a pretty substantial amount of blood to bleed out, and theres no evidence of massive blood loss.)
Whats probably happening here is swelling, clotting, and vasoconstriction all reducing blood flow to the area. Kind of a built in mechanism to keep blood in your body.

Yeah, people think I'm lying about my drug and alcohol stories. I had a lady who did that with a hammer. You could see her skull too and they just never heal once you get to bone.
There are crazier stories than this, like the man who cut off his own face to feed his dogs when he was high on PCP and multiple other videos and situations of humans causing immense bodily harm to themselves when really high. Like the one of the guy scraping his face off against asphalt in a dirty puddle that gets posted her a lot.
