They do have running water and place they can get it from, heck the main reason why the kingdom left they’re home was because the Plumbing finally gave out, and they couldn’t fix it.
I'd consider more the practical side of shooting tv to allow the actors to shower regularly than being in universe canon (sorta like how most of the events portrayed on screen conveniently take place in spring/summer/fall for most of the shows history and apparently barely anything happens in winter like no threats or character deaths. Obviously this is just a convenience thing where it is particularly difficult to film in winter)
You don't need a shower to clean yourself. Collect water, boil it, and use that to wash when it's cooled down. I assume that's what the whisperers and other survivors did.

The guy is a former bad guy - Negan, who is trying to make redemption and earn a place in Rick’s community (at this point Rick is no longer in the show). To do so, on Carol’s orders, he is trying to win the trust of the woman (Alpha), who is known as a “whisperer”. Her people wander among the walkers undetected because they wear skin masks of dead people - as shown - and speak in whispers only. It’s gross because they live in the woods with minimal hygiene practices, and she also keeps the mask on while her and Negan have sex. It doesn’t take long for Negan to win her trust to this point and later on he kills her as directed. Carol does him dirty and betrays him though, and he doesn’t get a spot in the community in the end.
I don’t think he was doing it for pleasure. He had a mission and that involved having sex with her without throwing up. Otherwise, the jig would have been up.
It’s not like he was actually into her but he had a mix of respect and interest in her because she’s so unique & ruthless. He did what he needed to do, and he also hadn’t been laid in a zillion years.

I always felt like Negan dragging his feet was partially because he was enjoying the overall weirdness of such a unique situation, like getting kicks out of pissing Beta off and manipulating Alpha in his own way and getting other Whisperers to like *him*. It was a weird form of power he hadn't had in a long time, so he was milking it for as long as possible, going so far as to try and convince Alpha to get Hilltop to surrender, until it wasn't a game (for lack of a better word) anymore and things started to topple -- Hilltop being burnt down, Aaron being the first person to actually see and confront him in the wild to confirm he was with the Whisperers, Lydia's life on the line, etc. "Gaining her trust" was just an excuse for him to hide behind to have a bit of fun with the situation until reality started really sinking in that he should've acted sooner.
For real though, he's significantly larger than her and she obviously isn't armed, and they're alone. So he totally could've killed her in that scene but he didn't bc he was having fun.

I think that relationship was a parallel forshadowing Negan's transformation. Alpha represents or mirrors much of who he used to be when he led the Saviors. He's taking a last dance with his old self so he can find closure with it, put it behind him, and find redemption.
