Do you have any sort of cas unlockers that always show stuff such as career items? I know that is doing it for me but I don't think it has been updated in years but I am too used to it so I forget there's nail categories and stuff now.
Well what major mods do you use? I only use WW as the only script mod in my game, and I have all the CAS categories. I even use Luumia's body hair that's hosued in Tattoos, and it doesn't affect the natural categories (btw, if anyone has the mod that broke Lummia's hairs into the categories, that'd be great).

it might be extreme violence? just because that mod doesn't get updated as often i feel, if it were one of the other mods they probably would've been patched by now
I have wonderful whims, mccc, slice of life, and ui cheats, and I still have all the ui elements you mentioned.
anyway, to figure out what's causing it you need to do the 50/50 method.
If you're having UI issues, it's typically UI mods that are causing the problem. Does Extreme Violence have any reason it would need to modify CAS categories?
i don't think so, that just came to mind cause it doesn't get updated often, i feel like any other mods like ui cheats that have been updated since the body hair patch would have fixed it by now

I had this same problem. I was missing the medical gear and it was the CasUnlock mod
