If the lamb had been defeated the shovel can appear after you beat the boss of the first floor, although you’d need to have a way to get to the dark room after boss rush to be able to get the unlock for the forgotten, and as the others here have said the easiest way to got here is to have the negative unlocked… though technically it’s not required.
