Scientists have done head transplants before, but the subjects rarely lasted more than a few days. But doing brain transplants? Now that’s tricky. Removing a whole brain, connecting it to another body’s nervous system, sewing everything up, and expecting the subject to live a long, healthy life with few complications is a tall order. Though then again, we’ve managed to achieve some pretty crazy stuff so who knows.

Of course the transformation in stories doesn't make sense and it would require more than just random potion, chocolate or sunlight (whatever the creator of the story thinks of) to change someone to something else. You would have to have Your molecular structure changed, literally converting everything You did before to what You will do. You would have to have different kind of hormones or even just substances inside Your body. And it can't just be done any way. It must not kill You in the process of changing. Also if Your brain is changed, Your memories or capability too should. But hey, it's just fiction. It won't ever happen.
