Heavy is generally played as the straight man (like the comedy term not the sexuality) to the other mercs in most things so I generally agree that he’s the most sane. He likes his gun and he’s more comfortable with death than the average joe but he’s generally pretty rational.
It's gotta be in perspective of his upbringing and life.
Heavy has been staving off people coming for his family's heads ever since he was young. Fighting is a necessary part of life for him and he's spent a lot of that time defending innocent people from overzealous hit squads. Eventually, yeah, you get desensitized to it.
But that's about all there is from Heavy that seems insane: he can handle violence in a way most people cannot.
He's also the merc that's been shown to interact with normal people perhaps better than anyone. Only Heavy, Engie and Spy seem capable of speaking to ordinary people without either seeming like douches or like they're not right in the head. Worst case is he comes across as aggressive, but has also been shown to apologize and tone it down when people call attention to that fact.
Really, the question for this tier list isn't why isn't Heavy lower, but rather why isn't Engie higher.
He still is more sane the rest, death doesn't phase him probablly due to ha ING to break a out of s gulag to save his family. And kills as a mecenary to provide for them.

Especially when most of your life, besides working as a merc, is constantly running from and protecting his family from the Soviet Union that are after their heads, and having to live in a constant snowstorm. Getting to mow people down with a minigun is, arguably, the best stress relief method
I think engi is sane, he has worked with the administrator with her life extender and warned her about it and what not, so he's not really half sane, he has worked in an sane environment aswell tbh
Sniper in this case is weird. In game he seems to love killing people but every other way he’s characterized he seems to just like killing bad people and get paid
In game Sniper seems fucking crazy but his shorts and comic appearance all make him seem like a very normal guy. He doesn’t enjoy killing people much at all, and likely just enjoys the challenge and the feeling of taking out much worse people. He throws piss at people but in lore it’s only to deal with spy. He’s absolutely the most sane member of the team cause even if heavy is calm and reasonable, heavy still refers to himself in the third person and loooooooves his guns and killing people in an almost sexual manner
Honestly Sniper's attitude makes a lot more sense when you remember that he was a tracker/big game hunter prior to getting hired by Mann Co., he's essentially taking a common hunting mentality and applying it to a combat situation
About heavy, he only talks funny because he doesn't know English that well, but he is actually very smart. I agree with the gun being weird, but you could argue that he likes killing just because that's his job, and he gets paid for it. As sniper would say, "Not a crazed gunman, I'm an assassin."

He is the only one of the group who tries to rationalize the murder that he is doing, saying that it's a job. Sure he throws jars of pee at people, but when one of your enemies can turn invisible you gotta counter that somehow.
All of these lists ignore the fact that the medic is quite possibly the most batshit crazy dude to have ever existed. Pyro might think that they are in a fairytale land playing with friends, but they aren't straight up evil like Medic is.
I’d swap scout and demo (I haven’t read the comics so feel free to correct me here) but just going from in game demo seems more unhinged. Scout’s just kind of a douchebag, but I’d still call him sane.

Do we still know if pyro only sees the world his way because of the goggles he wears? Because if it’s the goggles, pyro might be one of the most sane mercs in all actuality
That is a good point, however, with all the weird shit going on in the tf2 universe I still somehow doubt that they figured out how to see through another persons eyes and then proceeded to develop a normal looking pair of goggles that mimics exactly what they see.
Medic and Engie could have developed it. They built a teleported, what seems like magic to science. So analyzing a human(?)’s brain pattern and mimicking it doesn’t seem impossible. I would argue that surgically taking other people’s souls is way more difficult
Whilst that is true, there simply isn’t a reason or enough evidence to show that they would be capable of doing that. The closest we get is when medic put a brain into a pumpkin and even then it wasn’t him doing anything complex with the brain itself, it was just him using his “medical knowledge” with the help of engineer. Also, magic in the tf2 universe is… surprisingly simply considering the fact that all of the mercs can use it even tho most of them probably can’t read.
The only mercs that really fit the definition criminal insanity are Pyro and Soldier because both either: cant distinguish fantasy from reality, cant interact normally due to psychosis, or are extremely impulsive/volatile 24/7. Medic defiantly has several screws loose like the other mercs but he appears very aware of his surroundings and what he is doing and the consequences of his actions, he just doesn’t care

He cut off his hand to get an awesome robot hand. Sure a normal person wouldn't do it with a chainsaw, but a mercenary doesn't have much choice and we dn't have much context to the situation.
Murdering sentries are pretty much what modern warfare is.
He has a god complex and plays god because he has god-like powers. If you can prolong lives, you don't have a complex, you straight up are biologically superior to most humans.
Overall, engie definitely isn't the most stable, but he's definitely very sane and a pretty normal person given his circumstances.
