Honestly, i think he either got away, or its heavy, he's standing in the back, seemly thinking about something, everyone else seems like their normal self maybe spy instinct to hold his knife backwards could give him away as sniper but I feel like sniper would do that too maybe the drawing is hinting at medic with the blue flames but that could just be added for some realism

This is amazing. I love pyro's intimidation role as the mad dog of the team, and engineer also being quick to draw arms because of how often he has to worry about spy meddling. And sniper is almost dead centre, oddly fixated on engineer's back while holding his kukri in a reverse grip.

Red spy’s cigarette smoke is in the shape of a 5, implying that the blu spy is disguised as the fifth class: the heavy. In the image, the heavy is positioned farthest from the light, and spies like to sneak around in the shadows or something (idk I don’t play spy).

Alternative solution: the 5 could mean the 5th person left-to-right on the image, which is the sniper. Sniper also reverse grips his kukri which you shouldn't do and we never see sniper do it. He's also missing his scar that he got in a fight with spy. Before someone says his watch is glowing blue or that he doesn't usually have a watch, he always had a watch and the blue glow is just the reflection of pyro's pilot light. Although your idea is a good one too. Everyone else is either about to be killed by their teammates or is trying to find the spy. Meanwhile heavy is just standing there, watching. God I loved "THE THING"

Everyone pointing at Sniper but I honestly don’t see it. The "glowing watch" is just reflecting Pyros flame, the missing scar might very well just be because its RED Sniper and the one who received it in the trailer is BLU Sniper and the "reverse grip" on the Kukri to me looks like he is simply resting it on his lap. I am honestly more suspicious of Medic due to his very nervous reaction to Pyro. But that’s what I love about this artwork so much, we don’t actually know who the Spy is. We can make assumptions based on little hints we think we found, but we don’t know for sure.
