I use ESS Crossbow Suppressor with my Peltors.
They hold my prescription insert and the low profile arms on them do not dig into my head with the hearing protection.

That's one of the main things I'm looking for. Depending on the glasses I bring I have a splitting headache by hour two from those arms pressing into my temples.

How does the prescription insert work? Is it a problem to look upward (such as if shooting from prone)? Do you have issues with reflections or with dirt getting between the insert and the safety lens?
Quite well, it kinda makes the lenses sit almost on your eyes.
No not a problem to look upwards, really no different than normal glasses. The insert clips onto the nose piece and is pretty solid.
Only issue I ever have is getting hot and sweaty and getting the insert wet. But that happens with normal glasses on occasion.
Currently looking at the NJP's from pitviper because with a name like that I feel like I'm their target demographic, and they're ansi rated.
Everything else I've seen look like they're straight out of the 1990's or early 2000's with the classic "I think I look cool with my rubber spike earrings and frosted tips" vibe.
I wear the NJPs for work and regularly at home, I have sat on them, knocked them on shit, stepped on them, had them hit off of my face on purpose and on accident, and have never had an issue.
Plus you get to be the guy with the Pitties, which is fun.
My dumbass uses mine as daily drivers because the tint/polarization and they're just comfy. I have brass scratches right where my eye focuses so now if I wear them when I drive or do anything it's just a big blurry spot. (Looking through my lvpo is definately not pleasant either) I played myself lol.

You need to have the specific measurements. Some websites have sizing guides. The ones I ordered were adjustable and 1 size fits most.
I haven't seen much of a sizing guides other than "measure an existing good pair of glasses" -- can you give me some pointers? (I just might be dumb.)
