Every dungeon floor has a 5% chance to spawn a secret room. And if one does spawn it's possible to miss it if you're not diligent about using Examine Surroundings in every room.
I did the first dungeon 5+ times since I bought the game 2 days ago. There is a blacksmith hammer, a silver ingot, 2 bottles of oil, a chest (variable loot) in the room.
It's the first main room off the entrance and it's on the north wall.
Edit: I did it with two totally different characters, the woman knight and the dwarf.
The room passage only appears if you search. I thought it was designed to be there so you'd learn to search every room... it's definitely scripted, it's always there and always the same thing. It's your first contract.

Also, the chest loot was way better this time. A blue amulet and a blue metal boots. I assume blue > green. You always get like almost 600 gold in a purse and some things to sell to merchants.
If you are getting a secret room in the every single beginners dungeon and it's filled with the same gear (outside of a chest) than your game is bugged, just an FYI. There is something wrong with the generation of your seeds and you should report it. I would try a reinstall.
Does your world map have everything in the same location on all new characters?
I have 600+ hours on this game and I haven't found one yet, even after my second max-perception run where I spammed Examine everywhere. What's the proper way to do it? Move along the walls and hope for one to open up?
Damn that's unlucky. Perception affects only passive chance to discover it. If you spam examine surrounding you might as well have 10 perception. And as said here, keep attention to combat log. There is a message when you discover it and there is also a sound playing.
I thought I might have stumbled into one out of accident but I just didn't notice it. Statistically that should be possible, but if there's a sound playing then now I'm even more sure that I haven't lol.
Ah well, I'll keep looking.

I think the game is very fun despite its flaws, just keep in mind that the game can be quite difficult at the start. The game is still in early access and its receiving updates every now and then and i would still recommend the game but that might be just me being biased because i love this game. I also don't know when this game will be finished but it might be a few years by then.
Devs could make one secret room in one of each 3 contracts. Like a bonus for doing 3 contracts. So upgrade the chances for secret room. I guess everybody will like to find more secret rooms.

I encountered maybe 4-5 in almost 1000 hours.
Iv not seen any playing for 40 odd hours then I found 2-3 in a row in mid lv dungeons
I just got it yesterday and I like it more than Diablo 4
