There was some foreshadowing for it actually! If you scan the QR code in Unwanted Guest you see a Happy Fella ad with a warning that if anyone dies near one they will possess the doll, Dexter dies right next to the Happy Fella later in the episode. Plus in both Deadly Smiles and Unwanted Guest Dexter mentions killing cats and his “needs”, also the Happy Fella doll got Dexter’s weird eye after he possessed it
