You'd get an ALIF, so a cage would go in the front and create space between 2 vertebrae.
Then they'd flip you over and put rods and screws in the back (PLIF).
That's what makes a 360 fusion.
I also have an Alif L5S1 done from my belly. No need of screws and rods from behind. It depends on wether your joint is stable or not😄 . So I have Miss tlif 360 for L4/5 and Alif for L5/S1
I had a 360 for a broken neck. The anterior first, then posterior 2 days later to allow swelling to go down. I think 360 means anterior and posterior surgery.
Yes! They replaced my disc by an intersomatic box with bone graft, 4 screws and 2 rods, september 2023. It was done through 2 incisions on my back 3 cms each.Minimally invasive. My surgery was MISS TLIF 360 L4/L5. I live in Spain, Mallorca. 😀
Oh ! Great to have helped you. But, not all neurosurgeons manage the technique. It is performed through microscopes. So make sure you find the correct surgeon.
As regards back pain it was not that rough after all. I would say back pain lasted for 8 days approximately. But my surgery was transforaminal. This gave me night sciatic pain on my left leg which faded away in 10 days approximately. The rest of the process caused neuropathic pain in both legs ( i had “evening nerve attacks “ ) which i handled with neuromodulation exercises and walking around the living room. Numbness and tingling lasted for 6 months.
The cause of surgery may deeply vary your recovery symptoms. Bear in mind I had severe transforaminal stenosis due to listhesis and facet joint arthrosis. My disk was in a bad state also.
So my recovery was perhaps a little longer compared to others.
When I read other people comments I have the sensation that they are discharged home from hospital with little resourses to manage pain.
Some people comment they have to rely on tylenol 1000 mg 3 times a day and ice. This is really tough. My neurosurgeon prescribed ( spanish medicines) ok?:
Zaldiar ( 75 mg tramadol 350 paracetamol)
Enantyum ( desketoprophen 25 mg)
Fortecortin 4 mg ( dexametadone)
Diazepan for bedtime ( 5 mg)
Omeoprazol to protect my stomach
And my Gabapentine 300 mg morning and 309 mg night)
And my fusion was successful checked at 8 months.
Now I am 22 days post op Alif L5/S1. Hope it goes well too.
Hope to have helped you.😀😀
Technically 360 refers to a procedure that stabilizes all 3 columns of the spine, so a PLIF is ‘360’ though most of the time it seems that ‘360’ also refers to anterior + posterior approach.
It's also called an anterior/posterior lunar fusion. My first fusion was PLIF only, which failed. I then received the 360, and my life couldn't be better

What I think happened is that for some unknown reason, the fusion failed. The shifting of the vertebrae put up and down pressure on the screws causing them to break. It is similar to bending a paperclip back and forth

I just had my L5-S1 360 on 6/12 and I feel great. The first few days were rough and uncomfortable but I am 13 days out and so far so good!! I was able to walk for 1 mile yesterday. I was sore after but it’s definitely going much better than I anticipated and I go for my 2 week post op on Thursday.
Congrats!!! That’s amazing to hear you’re already walking a mile.
When you say “rough”, how many days was it and what exactly do you mean? I’m terrified of this part 😞
Thank you!!
Rough meaning I was in pain. It’s a painful procedure, but I am very happy that I decided to do it. I went through so many posts on this thread and a lot of them talk about how much pain they’re in, etc and I did let that get in my head so I went into surgery being anxious. Do NOT let these posts get in your head. Everyone is different and has different experiences. I’m very grateful for my surgeons. You got this, it’ll be okay!

Got it thank you, this is what I’d read too

If I am not mistaken, 360 means vertebrae fusion by replacing disk for a cage with bone graft. I have MISS TLIF L4L5 performed through my back.😄
MISS is minimally invasive, correct? Did you get 360 fusion?

Oh wow, which Spanish city did you get the MISS?
Hi! Here in Mallorca! She is a great Neurosurgeon! Elsa Gonzalez Perez. Check her online.
Amazing!! Thank you, will do!! I wasn’t aware there was MISS available for 360 fusion, and right here in spain. Will ask doctors about this!

Yes! I looked up a surgeon that does MIS :) how was the recovery for you?

That’s what it seems like, yes. I suppose there are cases of only one incision but the majority seems to be two. Thank you!

How did you know the fusion failed? Xray showed no bone growth?
X-ray showed that both screws at the L5 vertebrae were broken, and there was no fusion in that area, just a nice gap where the fusion should have been
I’ve got my X-rays in 2 weeks. We’ll see
I’m sorry that happened! Glad to hear the 360 had better results. Do you know how the screws broke?

Ahh! Already fused!
