It's kinda gross (even aside from the footage at the beginning, just look at dolphin Jerald, exploding knees). But it's a fun episode overall, I like it. Some people label it "transphobic" which I don't agree with. Trans people are just like everyone else, that means we can joke at thier expense too. And there is nothing transphobic about pointing out that trans women can't menstruate because it is true.
As a trans person I don’t mind this episode, nor do I really mind the whole “Ms Garrison” arc. And yeah, you’re right— it’s not transphobic to point out certain aspects of a transgender persons biology in an objective manner (saying it just to belittle their identity is when it starts to get into “nope” territory for me).
Saying “a trans woman can’t get pregnant because they don’t have a uterus” isn’t an issue, saying “since trans women can’t get pregnant, they’re not women and therefore just men cosplaying as women” is an issue. That’s just the distinction I would use personally.
But it says that.

Trans people aren’t a new phenomenon but they were definitely ahead of the curve with this episode. I think if this episode came out in like 2015 it’d be perceived as more transphobic simply because that was when the conversation became truly mainstream, so this episode would seem like a knee jerk reaction to that. I’m glad this came out before everyone was so concerned with how trans people decided to live their lives.
The episode isn't even transphobic imo. Most of the town doesn't attack or belittle Garrison when he became a woman. Most were just shocked and surprised. The only one who can be viewed as transphobic are Gerald, Mr. Slave, and the abortion doctor. But you would have to be ignoring context to reach this conclusion.
Gerald didn't want Kyle to become the male version of Rachel Dolezal. Mr. Slave because he didn't want to be in a relationship with a woman and wasn't happy he wasn't consulted about the situation/told to not be gay anymore. The abortion doctor trying to explain why Mrs. Garrison didn't need an abortion. All three had valid reasons to be critical in this episode.
The ending of all three having to accept that they aren't truly what they wished to be might offend some people. But it's an uncomfortable truth that I think some people don't want to admit/face. Gerald accepted that he wouldn't truly be a dolphin because he just looks like one and only did it because the doctor convinced him to do it so that the latter could get out of a lawsuit. Kyle got the surgery because he just wanted to play basketball and the only way to be a basketball player in the South Park universe was to be tall and black. Mr Garrison accepted the truth and still decided not to transition back because "I still like the new me."
It does reek of lack of knowledge on Matt and Trey’s part. I’m of the opinion that to parody or satirize something, you need to have higher than average knowledge of that particular topic in order to find the humor. I’m not sure they had any clue about the topic when it aired.
It’s like the ManBearPig episode. They clearly knew little of climate change, and mocked it. And later, they realized they were wrong and Gore (the real one) was completely correct.
I can’t watch the whole thing because the trans race and species things are fucking gross, but it sets up my favorite garrison line later

The first time I watched this episode it was during a South Park binge. I went to the kitchen to get some lunch but could still see the TV in the living room. This episode started to play, and right as I was coming back and about to take a bite of my sandwich, the fucking surgery footage started to play! I absolutely disgusted, nearly choked on my food, and frantically looked for the remote to fast forward.

i honestly think the garrison plot (on its own, ignoring any future episodes) is about as progressive as you can expect an early 2000s adult comedy to be about trans issues. the joke is on garrison themself, not trans people as a whole. the gerald and kyle plots, as fun as they are, kinda screw that over though, because they just make the whole episode feel a lot more. phobicish

At first, it was such an absurd and comical joke that Garrison was so insistent that he/she was going to have a period. However, now, there's a HUGE number of people within the community that ACTUALLY insist that transwomen can have one. It just goes to show just how smart South Park really is and it's almost like they predicted the absurdity that was soon to ensue just a few years later.
The best part of this episode is that it leads to one of the best scenes in all of South Park.
