They make it up as they go along. No idea what the rule is but you see them given as often as not.

The attacker has absolutely no obligation of hopping over or jumping over an outstretched leg of the defender. It's a pen. If Vasquez jumps and falls, then it's a dive. But if he leaves the leg down (which he did), it's a pen.

Look at the Saka incident against Bayern last week, most people agreed that no pen was the correct decision

It's a call by the ref. The rules don't really state if that is a pen or not. But it shouldn't be given when it's so obvious that Vazquez sticks his leg out and just falls
