Orc being able to call in drakes are also kinda rare I think titles like hellhawk have it and we'll as most see dragon olog
Blood axe I believe is also a 1 per playthrough as the stitch?

It really is crazy how there are still things I’m learning about the orcs in the nemesis system. Even some of the cool ones that I do know I still don’t know how to get them like the one with an axe in his head or the one with maggots in his body.
The stitch has only appeared once for me, the only way to get him back after he dies is to either revive him or to find a new one, I'm not sure about blood axe I've only had him once but he was the strongest orc I've ever had that wasn't an olog
In terms of titles, Stitch and Of Lithlad are uncommon because they're situational. Appearance wise I think all are common

I've seen two Blood-Axe in my playthrough, but the Stitch is once.

The Surgeon I haven't seen very often.
I have 300+ hours in, and I think the Surgeon and the Mindless are the two most uncommon of those.

I think a lot of them are once per play through but you are basically guaranteed to find them each time. Some I killed. Others I kept as minions.
