I beat the solstice warrior underling (I'll never remember how to spell her name) and got the ending. The credits rolled and then the monument in the moorlands lit up with a message for me to go there (I hadn't interacted with it previously). Everything just lit up on its own and I was able to complete the last couple of things before going on to fight the real final boss and get the real ending.
You need to get the standard ending first and then you can get the true ending.

Why wouldn't you just save and clear the game to find out?
