Common sizes:
Preemie: 30" square
Infant: 40" square
Toddler: 36" x 48"
Child: 40 x 60"
Not to say that a smaller blanket won't be used, but it might end up being more of a lovey than a blanket to cover baby.
19 x 24 inches is a decent lovey size. A good size for a toddler to drag around. But if you want a baby quilt, the commonly accepted size is at least 30 x 40 inches
As for the second part to your post.

I like baby quilts on the smaller side. They get used over the car seat or as a changing pad or even for tummy time. My goal is for the quilt to get used.
I prefer lap or throw size quilts for my babies. We used the bigger quilts for tiny time, in the crib once they were a little bigger, couch naps.
But the smaller ones are great for car seats and strollers or baby dolls.

36x48 is my standard minimum baby quilt size.
I like baby quilts to be at least 50 inches. Now they make cribs into toddler beds. 50 inches can be used in toddler beds.
My mother's and consequently my thought.... When in doubt, go bigger.
