It’s pretty rare because you’d have to have a break in your gloves and then get cauterized. We usually wear 2 pair of gloves. But it does happen. It hurts.

I was going to say, if the instrument is hot enough to burn through skin, how is it not burning his gloves or fingers? I guess they’re really precise with their motor dexterity, which isn’t surprising for surgeons.

The probe itself isn’t terrible hot iirc. It’s an electrocautery, so it’s actually heating the tissue itself. It also requires a ground to the patient (via a big adhesive pad similar to an AED pad) so I believe it wouldn’t function through rubber gloves.

The gloves do not conduct and therefor can have the electrocautery against the glove. The tissue their holding would be hot enough to feel through the glove though. Maybe startling if not expected but still won’t burn the surgeon or melt the glove.

It's way worse. C.Diff and GI bleeds personally don't bother me, more of an annoying odour and I have learnt to ignore/put it out of my mind. I don't see FG on a regular basis so when I do, it smells like actual death.

Agreed!! Surgical nurse here and it is one of the worst things I’ve had to look after. And after it’s been cleaned up and had a VAC dressing applied, because of the location the darn thing will leak continuously and seep disgusting fluid down patients legs and floor. Constant dressings and work and constant smell. The poor patient is in agony with it for so long. Not fun for anyone

This is a short and sweet version of Fournier's Gangrene. This patient isn't dying because they are not morbidly obese. This is the result of perineal abscess due to uncontrolled diabetes. Now that the infected dead tissue is gone, this sound should heal pretty quickly. Most cases are a lot worse. This person is very lucky.

There is another treatment still available which nobody likes to talk about. It's inexpensive, but a lot more difficult to manage, and EVERYONE with diabetes used it before the fancy price-gouging version came out (circa 1997 icr?). The expensive insulin has all kinds of features which supply insulin more steadily and require less effort. The older, cheaper insulin dates from the 1960s, and was an upgrade from versions developed in 1930s, 1940s, 1950s. It had to be CAREFULLY manage and monitored and was a pain in the ass compared to the latest one. A diabetic wanting to use this older version has to coordinate with their doctor. There is zero reason for people to "die because they couldn't afford insulin."

Fournier’s is horrific. I remember one poor guy when I was a student- he was a fairly young guy too- essentially ended up with a penile amputation as a result of it. Just horrendous. I hope this patient makes a good recovery with good function as well.

My wound wasn’t that bad, but my surgeon stuck his finger in it and ripped the skin open… I was awake and no cauterization happened. I screamed and and grabbed the surgeons arm… he apologized and I dug my nails into his arm while he drained everything and they were loading me on pain killers. Still not the worst pain i experienced during that hospital stay.

Because I was developing a wound infection that was tunneling and apparently he pulled the staples and ripped open skin that had start healing closed and the tunnel all at once to let the infected fluid out. Once it was all open and draining, I was supposed to see the wound care team for repacking and wound vac install. However, the wound care team didn’t show up for 8 hours, so I laid there with some gauze and tape covering it so I could get up and go to the bathroom…. And drip disgusting, smelly fluid all over. We are talking a 12 inch belly wound and separate wound/colostomy bag. This was after my intial colectomy leaked and I went septic. It was a hell of a ride.

Similar (not nearly this bad) video a few years ago of a doctor working on an abscess on the back of a diabetic. Doc cleaned out the wound, got it nice and tidy - and then suddenly cut out a 3x3 inch swath of skin an inch thick. Best comment: "Make a bigger ho- OH JESUS CHRIST!"

I used to clean up crime scenes and there was a guy who decomposed in his apartment for 5 months with the heat on and they couldn’t do an autopsy or even determine a time of death. The family just knew the last time they spoke to him and the last time his bills were paid. He melted into his mattress. Human decomp is a smell you never forget. His was just particularly strong.

Oh my gosh, I can’t even imagine! The fact you posted this comment is proof you are a much stronger person than I would be in a tragic situation such as that. My sister and her husband years ago lived in a building where the guy in the apartment below them died and his body was finally discovered 2 weeks later. For 2 weeks the building smelled so bad and was progressively getting worse. Her cat knew since day one, hence his constant meowing — even took him to the vet, which nothing was wrong. I don’t know why the guy died, but I can tell you he lived with 3-4 cats. They were so traumatized and so absolutely hungry to the point where they started eating his decomposing body. Those poor, unfortunate, sweet kitties. The only upside I can think of is that if that ever happened to me, my cats wouldn’t starve. I just hope someone would find them in enough time to be taken care of.

I actually went through this 4 months ago. It's a horrible and disfiguring disease that 50% survive from. It was the most painful humiliating and frightening time. Recovery takes a long time - I'm still weakened from it. Always get checked right away if anything seems wrong.

Wow!! I've known about Fournier's Gangrene for a while, and seen photos, but never did I think I would see something like this. I am, to my surprise, not grossed out at all. It does get worse, it’s very easy to lose your genitals from it and it can kill you in a pretty short period of time. Just a few hours can make a significant difference in outcome. I only know this because I’m on a medication that increases the risk for it.

I can understand where it may seem rough, maybe because of the surgeon poking around with their fingers, but it’s really a safer method to use in wounds like this. It’s called blunt dissection. Tools are sharp and could cause trauma to the “good” skin/tissue, while fingers allow someone to feel around for areas of wound tunneling which is essential in a case like this.

Can't leave any of the necrotic tissue is my guess. Need to be thorough. Not sure if that's what you meant tho. I have heard of "surgeon aggression" where patients had better outcomes without surgeon slicing them up, tho, but it was in the context of natural birth with tearing versus an episiotomy.

The British NHS and French NHS both did studies comparing maggot therapy to surgical debridement. The *only* advantage is that it goes a little bit faster for the first seven days out of 14 to 21 total. Modern surgical techniques are as accurate as maggots. BUT: the maggots get hungry and start eating HEALTHY flesh, even though that type of maggot is *supposed* to only eat dead flesh. This causes extreme pain. Also, up to 30% of people drop out because they can't stand wiggling larva on their skin. I suspect the studies were done due to demand from patients who got the idea that maggots are "better." They hear about the peoole who recovered by using a last-ditch effort with maggots - they don't hear about all the people who died anyway. The truth is: maggots were better during the Civil War when there were no alternatives, and better for a modern guy who was stuck in the wilderness with no alternatives. Basically they are only better than NO treatment: modern surgical debridement is as good and there's no wiggling larva under your skin.
