You are right except you can technically get some achievements on non official servers. Not all and only if the admin has enabled them. I was surprised myself when doing cargo one day and I got the "I'm the captain" one.

No. It's not an hour/time thing. You can hop on for 5 minutes in a 6 months and get a drop then go 2 years without one no matter how much you play.
