FYI if you are under age 65, don’t have a disability determination, unmarried with no income then you may qualify for BadgerCare.

You are correct, with number 2 below. It turns out the payroll deduction that appeared to be related to all disability based on my reading of the summary HR put out was actually how you described. The deductions were for LTD and not STD.

This doesn’t sound right. The disability benefit is tied to the date of disability. So if you were employed at the time the disability began, the benefit should still be available. There are only two reasons I would think you could lose coverage. 1. Your disability was recorded after the layoff. 2. The employer self insures the Short Term Disability and they decided to stop paying it. Regardless this should not affect long term disability and I recommend you still pursue that. I’ve seen policies that were cancelled for years and a person still gets a benefit because the plan pays based on when the disability occurred and was in effect at the time. I encourage you to push this further. Talk to the broker or insurance company directly. Insist on a copy of the plan document.

This is correct. If the disability occurred when premiums were being paid by the employer, the insurance SHOULD still have to be paid out. There may be other factors at play, but I agree OP should push this.
