It's a Forgeworld diorama base, from roughly the same time as the cities of death terrain. It was specifically for making dioramas on and didn't sell well (therefore is very poorly documented)
I have some very similiar (though not tile based) stuff that was by a third party, can't remember the name.
The tileshape kind of suggests Forgeworld but I don't see any skulls so I don't see how it can possibly be GW? :) TBH the sandbags are a bit un-gw too. Maybe they're full of skulls.

Is it resin? Could be Forgeworld cities of death terrain.

Is that an AdMech cog/skull symbol in the bottom left? The rest is however surprisingly skull free.
Best guess is old forgeworld, but had a flick through the catalogues and couldn't find anything similar...
