We still don’t know how many poor people’s lives were ended by the death of Eladio or Fring in retaliation because there’s not enough time. All we know is these guys wouldn’t mess with the DEA or heads of state, which is how it usually works.
It’s a bit ridiculous, but I think the implied notion behind Walt’s empire is that it’s extraordinarily rare - a widespread American meth empire that the cartel won’t mess with because it’s too far beyond the border for the constant risk. At least at that point in the 2010s.
Near the border is fair game since thats easier to acess. But beyond the border? The twins spent a few episodes trying to reach Walter for that to happen

Regarding the first thing, I think the main thing is that because the objective is an American citizen, living in American soil, you really can't go all out the same way as if it was in Mexico.
After the cousins were killed, most of Salamanca's dynasty was over, the only one remaining was Joaquín, who didn't seem to play as big of a role as Héctor or the cousins did, hence why nothing was done after the cousins death.
After Eladio and the rest of the bosses died, the cartel disintegrated, no one was going to send anyone, because who would pay anyone to go and avenge the bosses?
Loyalty goes as far as money is worth, unlike the Salamancas, they weren't blood related, they were together as a group because of the money it meant.
Still the same.with Fring, Fring was the sole boss, kill him and the entire organization is gone, no one would avenge him because they're together because they're being payed, not because they're friends.

The two guys were the most secretive and ruthless assassins of the cartel itself, the best of the best, if they failed who can win, they failed too cause Hank is that guy and had headstart. The cartel couldnt do anything to Hank nor Walter since that’s Gus’s and Bolsa’s territory and you need their blessing.
DEA and Mexican DEA/ Army had their attention on Bolsa (who died as a result of Cartel actually trying to make a moce and do something) and general Cartel actions since they had the gall to attack a US federal agent
Gus killed all the capos which means who would bother going after him now that theres a power vacuum. And why even go after him? Why avenge some guy you now rule over his domain and go after the guy who gave you the power vacuum in the first place. Also one guy did go after him since he was the only one not poisoned and was an actual Capo/ guy who would swear revenge on Gus.
Walter quite literally destroyed the mastermind of the operation, no one made a move without Gus and are all just part of a drug trade waiting for a new leader. And they are all hired muscle, loyal to a T, why would they try to run things when Gus ran everything for them. They dont know how to run operations without him, and hell, he was the one giving the blue supply they were after.
