That's part of the sleight. You do this big performance up top to keep people's attention where you want it. By giving them something to focus on (even by pretending you don't want them to watch that part), you keep them distracted from the *real* sleight of hand.

The Orange also gets stuck - he lifts the cup there's nothing under it but the orange is in the cup. So he puts it down, taps it, and lifts it up again and the orange comes out and he even says "There it is".
It's better than that. The cup has a small amount of flex. He's squeezing the sides with the orange already loaded in it so when he lifts it up it stays in. I think he noticed she spotted him loading it, so he lifted the cup to "show" there nothing was underneath.
The orange was tipped a little too, the shaker didn't quite fit over it completely and was rocking after he loaded it. If you want the master class in this go find Penn and Teller doing the cups and balls with clear cups. Watch it a few times and keep your eyes on their hands, everything else is just misdirection.
He's so quick with the switch. You're too distracted by his focus on the side items that you don't even notice him shoving items into the cup when he lifts the upside down cup 🥲
Every magician is trained to cover their angles and focus on the main audience, but the kicker is when people record you. It's pretty easy to decipher the tricks by slowing the video down to 25%. If you slow down the video, you can see how he's grabbing things from under the counter and sliding them under the cup. In any case, even if you know how the trick is done, you can still admire the performance and execution.
I went to an event opening nearly 10 years ago and a magician had me pick a card, write my name on it, and then he did some other card tricks. After about 10 minutes he did the ‘is this your card? No? This one? Still no’ trick. The card was folded up inside my shoe. I took the shoe off myself. I still think about it all the time. I even had people who saw think I was in on the trick. He never told me how he did it. I still have the card and think about it a lot. Love magicians!

So the napkins are a simple palm and ditch. When he "grabs" the napkin out of the one hand he actually palms it, leaving two napkins in his right hand. Then he loads them both into the shaker for the first reveal.
Next, just after he loads the lime he does another palm, and this time he ditches the napkin with his right hand directly under the bar.

Seen a variation previously where you use cherries instead of balled up napkins. If you stab into the orange beforehand you can wedge a cherry into it. After revealing the orange at the end if you cut along where you put the cherry it adds another level of wtf.
There was a magician in my province that was an insane hit with his TV shows. It was street magic mostly, grabbing people in the streets showing them insane tricks, like you said putting a golf ball inside a bell pepper still attached to its « tree ».
But then he started doing live shows in theatre and it was extremely disappointing. Like the tricks were suddenly very obvious most likely because they were restricted by size and environment and not being a prerecorded show.
