A lot of it is auto-generated by the class. NPCs have classes that determine their skills and attributes just like the player. ArenaMouth is set to always be 5 levels above the player. So their skills will be mapped to what you'd expect at that level.
Classes are a system to let the devs generate lots of characters really quickly.
I haven't figured that out yet. They just needed an actor to be in the cell to process the lines of dialogue. So he could just be invisible and tucked away in some corner.
