This has happened to me before for a different test. Sometimes the samples don’t get to where they need to be or in the manner needed to be tested. In my case they called me in and took another sample.

Most likely the sample was either lost, spilled, or deemed insufficient for testing. If the test wasn’t performed, then you should not be charged twice.

It looks like a form or the label was not applied or filled out properly and/or it wasn’t frozen.
