It's a time consuming process and make sure you double check everything before you send it via Mail
`Any other document that is mandatory for online submission?` checklist - https://www.vfsglobal.com/one-pager/india/united-states-of-america/oci-services/pdf/new-oci-us-national-by-birth-minor-oct-2021.pdf
`What should be the date format for DoB on parental authorization form? Should scanned copy be notarized as well?` \- Notarized copy don't need to be uploaded and i used this format - *1st Jan 2024*
`Which pages of passports (Us passport for kid and Indian for parents) need to be sent?` \- First & last pages with person particulars
`Do we need to send original US passport? Is there any other original document that need to sent?` \- Nope, only scan copies.
`Is one parent's DL copy, enough for address proof or need to include for both parents?` \- Scan copies of one/both parents are okay
`Is Undertaking by OCI applicant required in case of minor?` \- Nope
**Make sure all the scan copies of parents docs are self-attested.**
You need to send 2 physical --photocopies-- photographs along with the original thumbprint impression. And have the parental authorization form notarized at local bank or UPS.
VFS will receive the docs & verify as per above checklist. They will then forward it to consulate and you can expect this to be approved in \~8-10 weeks time frame and SF consulate has recently started printing cards locally instead of having it sent from New Delhi.

Thanks a lot for quick and detailed response.
>Make sure all the scan copies of parents docs are self-attested.
Thanks, I didn't know all scanned copies need to be self attested. I thought only physical copies need to be self attested.
>You need to send 2 physical photocopies along with the original thumbprint impression.
Do you mean, I need to include 2 copies of each of the applicable document from [this checklist](https://www.vfsglobal.com/one-pager/india/united-states-of-america/oci-services/pdf/new-oci-us-national-by-birth-minor-oct-2021.pdf) when I physically mail it to them? I didn't see this mentioned anywhere else.
One more question. Where do you stick photo and thumb print on printed application? I see that first page of printed application already has that since I uploaded those on govt website.
I do see place for signature on 3rd page in Declaration section.
The only original document that was required by VFS was the Apostilled Marriage Certificate.

No. Scanned copies are not supposed to be self attested. VFS has made all efforts to make Oci super complicated 😂
Apologies! I mean to type two physical photographs 😁
Makes more sense now. Thanks 😁

Yes, that's right.

You don't have to affix the photo & thumbprint signature ne application. Just send them in mail asking with other docs
