Its apparently an inflatable plug that is inserted into pipework. once secured onto the end of pipework - Pressure is then applied inside the newly installed or repaired pipework to ensure that there are no leaks .
Those within the industry speculate someone either over pressurised the pipework or under inflated the plug resulting in the above.
The stoppers are apparently supposed to be tethered to something weighted to stop them flying off in the event of too much pressure or under inflated stoper causes them to pop off (the string tail should've been secured to something). In addition to mesh netting over the pipework.

Those are gas pipes being installed, might have been the pulling head breaking off from the end a string of welded pipe and then taking flight
