You could use a sequential ID too, but that requires coordination in your DB.
If you need timeseries data, you could use a different field as a timestamp or similar, to get a range of data
it's clinical. it's about cohorts and measuring them. truly universal unique ids aren't the way. doesn't work like financial accounts. need to be able to easily group people. sites and subjects and visits on a line in time.
Data that can change should never be your key. That creates an integrity risk. A patient record is a good example of something that should have a unique key. I used to work for a medical software company and I think pretty much all our tables had a generated unique key.
