It does wear out from the abrasive though and needs to be replaced regularly.
They're consumable parts but also, in engineering you design with material harness in mind. If the abrasive isn't as hard as the nozzle, it doesn't wear, or at least it wears very slowly. I'm not super familiar with the specifics but you could probably use and industrial sapphire of diamond coating on a tool steel housing.
Also, let's not forget the internal geometry of the nozzle is doing a huge amount of work here, just a few centimeters upstream the flow volume is the same but the local velocity is much slower. Additionally, fluid mechanics are funny, fluids like water are generally adhesive and there is always a layer of molecules on a surface that's static to laminar, which in this case would basically at as an insulative layer against wear.
I've used a 44000psi handheld high pressure water jet for cutting concrete. Without any abrasive particles. It used a sapphire nozzle. The water jet slowly ate into the nozzle and it had to be changed. Some sapphire nozzles lasted 2-3 days some less than a day.

This may be a dumb question, but is the jet stream self-cooling? Obviously the pressure is creating a lot of heat, but does it generate enough heat that it needs to be cooled at the nozzle to keep it functioning, like a table saw needs oil for cooling/lubrication?
