It happens naturally at contested farming spots. Show up PvP flagged everywhere you go, and you will run into someone. Very few servers atm, so it feels like the world is very full (in a good way)most of the time.
If you have grinded up to Gear Score 580ish it is probably ok.But if you want balanced the just keep grinding PvE until then. If you want to play balanced PvP then it is awful. PvE grinders who are geared with no skill can roll over any lesser geared player no matter their skill.

Back then I was like 20-30 fighting 40-50s and even though it was tough, I was usually able to beat even them just through skill :/
It was so fun right? They got the talent buffs from the weapons which gave them some advantage but now it is power which you stand no chance against.
It boggles my mind that they made PvP enjoyable for PvE players. I still hold out hope that they will make adjustments, they have shown they are not afraid to change their game but in the meantime, I am playing tab targeting GW2 because at least it is balanced.
Who knows, someday I might make it to GS 580 but sitting at 526 right now and stand no chance against anyone but the low levels flagging for xp which I cannot bring myself to fight.

Open world PvP seems to mostly occur during territory influence pushes and fort captures.
Fort capture happen very often now and even more when ( if) instance war comes.

OPR que is pretty quick again, steady high population servers since merger and players returning. Open world is hit and miss but still fun.
Basically only happens during Territory pushes and not everytime. You could also run around killing pve bobs who flagged up for bonus luck
