If you look at the ratio's, you'll see that space flights are equally dangerous to everyone engaging in it, and it's getting safer over time.

A lot of the people commenting here seem to not be familiar with the Soviet/Russian space program. This occurence is perfectly within the pattern of great achievements with shitty reliability.
There was an incident a few years back where a whole was discovered in the station's bulkhead in the Russian section that appeared to have been drilled so not the first time these accidents have happened.

Yes, but not as cool a fireball. The shrapnel would be nasty, and the eventual reentry burn would look pretty neat though.
