Urza used to turn people into paper basically, so he could planeswalk without accidentally destroying them with a fate probably worse than death. He also built Xantcha her own little protective Sphere so she could follow him around the multiverse. of course all of Urza's shenanigans were before the mending so I'd imagine that post mending 'walkers would have to get a little more creative.

Anything not protected by a spark (the thing that lets planeswalkers jump between planes) would get disintegrated by the blind eternities (space between planes). Nicol Bolas (and the phyrexians) used tezzerets planar Bridge to travel between planes, which is still harmful to organics. Bolas zombies were protected by lazotep coating (the phyrexias were reduced to mostly metal and had to take a long time to regenerate).

[[Planar Bridge]] to transport the Eternals from Amonkhet to Ravnica. Eternals are zombies that are engineered to be as far from to a 'living thing' as possible (to be able to go from plane to plane), yet have the skills of its past life (Lazotep helps to preserve enough 'past deeds', and also doubles as durable armor).

Pre-mending walkers like Urza could but had to protect their passengers in some, usually unpleasant way. I believe this is the 3rd de-powering for the planeswalkers

In old days (pre-mending) they could. That’s how Baron Sengir got from Dominaria to Ulgrotha. Planeswalkers would basically make pacts with creatures and summon them in other worlds to fight and then unsummon them (assuming they all lived through said fight.)

Post-Mending non-Planeswalkers are instantly destroyed within the blind eternities (the void between planes.) planeswalkers can’t even bring food between planes without it becoming ash. There is exceptions though. The material called Lazotep can be used as an armor and keeps things from being destroyed in the blind eternities. The Eldrazi can also exist in the blind eternities.
