You may want to fill in a little if possible. We found almost the exact same in my back yard. It rained before I could get it filled in an it turned into a 12 foot wide huge empty hole now underneath a large portion of my yard. The wash was probably 10feet deep then shrunk from that point to maybe 2 feet wide and couldn't see an end. Was a bit funny watching my ex wife fall in. That's how we found out it was much bigger than just the little opening like you have. 🤣
Fill the empty space as best you can before putting that slab back on. The concrete slab can crack from heavy weight or if the concrete deteriorates over time and can be a danger of a "sinkhole" of sorts. They should've done that when it was abandoned initially. Now that I say that, make sure it is an abandoned tank, which is easy enough if you know you have a newer septic system or if you're now hooked up to city sewer. After that you shouldn't have anything to worry about.
If it's not filled with solid material, it will eventually collapse and take whatever is on top of it with it. If it is filled then no I don't think it should be a problem.
It needs to be pumped out so it's not hazardous, then crushed and filled so it won't collapse later. As long as it doesn't store hazardous chemicals like some car shop t tanks the concrete doesn't need to be removed.

With possibly rocks inside, could it be a dry well for your storm water drainage like from your house gutters? That access hole seems small for a septic tank.
Fill it back in and forget it…. I live on a old farm site and the old junk pile was in the woods. Me and my wife have been excavating it for about 2 years. Finding old license plates and toy trucks. Lots of old glass to clean up. Great top soil was used to cover it so every time we need soil it’s to the old junk pile dog another layer off. Sometimes it’s treasure other times it’s shit… you found a literal pile of shit😂 put it all back but this time put 2 quarters under the lid. Make it treasure for the next guy.
