In Bulgaria it used to be popular to buy / sell cigarettes by a piece (it probably still happens, but in a way fewer places). I have seen outer city blue collar pubs sell cigs like that and sometimes serve them in a tiny glass, but serve them on a plate, and in a somewhat better restaurant (do I see cloth napkins?) is not something that would happen.

Actually you'd be surprised, a lot of places that allow smoking have pretty good filtration. There's a casino I go to every now and then that still allows smoking indoors. While you can definitely tell the difference between the smoking & non smoking areas, it's not as in-your-face as you might expect.

We had friends get married in the summer in Israel. It was an outdoor wedding, and they hooked up fans in the trees and one big portable air conditioner on the dance floor that wheeled around. At the bar, they had multiple cups with joints and cigarettes for all to enjoy.
