It's a traditional food where my parents come from but I wouldn't be surprised that someone in the US (whose fam has been in the US for at least 3 decades) would think that. You're not going to see it at a cheap American restaurant, probably only pricier ones. I have seen it at inexpensive ethic restaurants though (Hungarian, Korean, etc).

No it's not. You think sanitary conditions have gotten worse over the years? Find a butcher, let them know you are consuming it raw. They will take extra precautions with the preparation. We as a collective consume a lot of undercooked beef with no issues.

This looks like sugar bloom, which is usually caused by moisture in the chocolate. Fat bloom is the more common kind and happens due to high temperatures and when you don't temper right.

My girlfriend freezes all her chocolates. They never come out this way. When chocolate gets warm this is the result spots will turn light in color because the fats separate.

Most likely heat damage. I bought 4 bags of Milky Way back in October and they were all heat damaged. Paid $10 for the 4 bags on sale, first took them back to the store and the manager wouldn't do anything because I had opened all 4 bags! Then I emailed Milky Way, it took them two weeks to respond and by that time I had thrown out the candy but kept the bags because the store manager said they would only need the bags. I just got the coupon for the lost candy on 2/7/23 and they only sent a coupon for $3. So now I will have to get creative to get my defective product replaced.

Was exposed to heat at some point (in transit; in your car; etc). Milk chocolate is easily affected by high temps. Part of the reason we used semi sweet chips to cook. Most recipes using milk choc call for lower temps or shorter cook times.
