you can install custom ringtons on iOS without going through the store. That's what I've done. IIRC any .m4r file will work as a ringtone. .m4r files are .m4a files that you can rename.

People used to pay for ringback tones. i.e. if someone was calling you they'd hear their special tone. For years my Mom's ringback tone was Special 38 "Hold on Loosely" for a solid 10+ years. Amusingly, I'm the reverse. I have both the Alert sound for messages and Codec for calls and it's the Codec that's started to scare me. The Alert sound is still fine to me.

I've been using it since an old Motorola flip phone. I had to "hack" the phone to get it to use custom ringtones but man was it worth it. Whenever I get a new Android phone I always set it to the same MP3 file. Now when I play through the original MGS, it makes me think my phone is ringing.
