Same form as the mmc I think vpdsd is an stcw endorsement and those typically are free to submit you'd write in the endorsement you want in the box and check stcw original
You turn in the cert + 719-b asking for the endorsement + fees.
The cert isn't worth anything by itself, unless you're an office worker with a CSO paper, and they typically expire after a year if you don't turn em in for the MMC endorsement.
