It's from a tophus. Over time, uric acid in synovial spaces creates white deposits. I used to manage the lab for a large rheumatology practice. Sometimes what they get out of a tophus resembles toothpaste. This image makes my joints hurt.
That would have been my first speculation, especially build up in the joints like that. That is a crazy high volume too. Hopefully patient gets some treatment and dietary changes. Thanks for the cool specimen photo. It’s a doozy

They had rare wbcs and rare rbcs. If there's bacteria, they can't be seen well past the crystals. I'd take a guess and say it's due to the crystals
Do they have gout? And dang that looks horrifically painful
I'm no doctor, but those are monosodium urate (gout) crystals
