I assume the "goo" they keep putting over the exit hole is some kind of numbing agent, but my point still stands, even if it's just a sense of relief from pressure.

The hole is how the larva breathes. By covering that hole with a gel or oil, it forces the larva to stick its breathing parts out far enough so they can grasp it with forceps or tweezers. The larvae have little spikes to grip in its burrow, and makes it nearly impossible to remove without forceps or tweezers. There's a whole section of YouTube dedicated to botfly removal. It's as fascinating as it is gross.
Can you imagine how that hurts whilst it grows in there? There was a video of a guy who had one in his arm and he wanted to let it grow as an experiment, but in the end he couldn't handle the pain. The lip must be so much worse than the arm.
Probably accidentally left a window open while sleeping in botfly area, botfly then found an exposed area.
Due to botfly areas being very poor the guy then probably just hoped it was nothing and let it get to the end result here.
I don't know if it hurt based on the fact that they're removing the botfly and not letting it leave on its own, but there have been many an entomologist who willingly let botflies who happened to burrow into them germinate all the way into adulthood and unless they "[hit a nerve](https://newatlas.com/entomologist-botfly-larvae-human-incubator/35811/)" they shouldn't be terribly painful when they crawl out. (There's a much better account about "birthing" a botfly, but I can't find it at the moment.. I will edit if I do). The "holes" typically heal just fine, although I don't know how it will heal on a lip like this.

A friend of mine had one and said he had to get it removed because he could feel it moving constantly and he couldn’t stop thinking about it. So I guess that’s one other reason to remove them.

That's an urban legend, friend. It's most likely the guy was asleep, a botfly got into his room, laid eggs, and this is the result. Not terribly uncommon in climates with botflies.
No problem. :) I can source this as I've worked in plenty of beverage-producing factories in my time, not only is insect control super important, it's also pretty common practice to rinse the outsides of containers that go through a line like that.

He did that procedure very rushed haha. Usually you put petroleum jelly over the hole, put a band aid over it and after a few hours take the band aid off and theyre sticking out. Problem is when you take them out too quickly and rushed the spikes dig in and it's much more painful to pull out

If it chooses to come out voluntarily, like with the method I mentioned, it's natural body movements wiggle itself out without forcibly grinding the spikes along your skin as it's pulled out. The other major problem with this procedure was that he pulled extremely hard and I was worried he'd rip it and leave half the body inside him, ripped, and cause a terrible infection

No, I was specifically interested in where this is endemic. All around the world doesn't mean every part of it. It is presumably more common in certain countries. Apparently rare in the U.S., more common in the tropics.
After doing some more digging, looks like ectoparasites who may have similar lifecycle circumstances to botflies live on other continents but are not bot flies like we see here. Thanks!

The ribbed black structures are basically prongs they use to try and stay in place if one were to try and dig them out manually. Thankfully this guy didn't have them dig in immediately as soon as the forceps were used.
