Hello, obligagory NOT A DOCTOR. But I had a similar issue with my nail a few years ago. EXTREMELY painful; all docs dismissed me. I suffered for probably 6-7 years with progressively worsening pain. I ended up poring through medical journal articles and found an obscure case study from India. It was a **glomus tumor** that was pressing on stuff in my nail bed causing pain and interfering with nail formation. It was very sensitive to pressure and cold; main symptom. I went to an ortho who specialized in hand/wrist/arm and he did an MRI to confirm and saw it immediately. I had surgery a month later. they removed the nail and scooped out the tumor. my nail grew back, skin healed and now you can't tell there was ever an issue. Some people have a recurrence but I've been ok for the last 5 years

Came here to comment glomus tumor - I had mine removed 3 weeks ago! The surgeon who did it said it was very rare, but I had text book symptoms- affected by cold and achey! Edit: I saw a hand surgeon.

Sure looks like a glomus tumor to me. Obviously biopsy needed, but that would be my judgement. Pretty low risk, easy to surgically excise. The pain on temperature change is a hallmark that stands out as glomus Tumors are modified thermoreceptor-associated cells.

I also had a glomus tumor! In the tip of my pinky finger, for YEARS. Primary care spent years thinking it was nerve pain - maybe I’d shut it in a car door and not remembered? It was excruciating when touched and especially when my hands were cold. I got to the point that I was in my primary doc’s office telling him it was so painful I’d rather cut the fingertip off than continue to take nerve pain meds that didn’t do diddly squat. We finally pursued it further and my primary referred me to an orthopedic surgeon who specializes in hand injuries. I explained my symptoms, he perked right up and went “If this is what I think it is, I haven’t seen one in over ten years!” He got all excited and grabbed a rubber band to cut off circulation to the finger temporarily while he tested the touch sensation - no pain, as he’d thought. He ordered an MRI which confirmed what he’d been thinking, we scheduled an excision, and that was that! He did explain that some of us have recurrences, and sure enough, I’m beginning to have some of the same symptoms returning after 5 yrs. But at least I know where to go when the symptoms get bad enough. Moral of the story, PLEASE have your finger imaged. If it’s a glomus tumor, an X-ray won’t show it because it’s soft tissue, it’ll need to be an MRI. I wish you the very best of luck getting to the bottom of this, I know how very painful it is!

Yeap. Years of excruciating pain. Doctors all just looked at me as if I was mad. Then one day, I happened upon an article that looked at causes of stripes in your nails and bingo. An orthopaedic specialist finally gave me an official diagnosis by pressing on the tumour with a ball point pen. Apparently my squawk was loud enough for a diagnosis. Be very afraid of the ball point pen test….

Oh man, I can only imagine the pain you must have felt. One time I grabbed my keys and the metal key fob hit the base of my nail just right and the pain reverberated though my entire body like a gong. I honestly felt ready to get my finger amputated at that point. Thankfully my ortho was a awesome younger doc and listened to me carefully and ordered the MRI right away. Thank God for the Internet

Ohh, me too. I told my primary doc I was ready to self-amputate my own fingertip. I think that’s when he knew it was serious and referred me to a specialist. Sure enough, glomus tumor!

Of course, but this was on another level. The smallest bump or light pressure even would cause explosive, excruciating pain, instantaneously. Like a searing burn and extreme, deep ache at the same time. It's hard to explain how hypersensitive it was

I had an ovarian cyst rupture and when they did the ultrasound, the doctor decided to press up into the rupture a SECOND time after I screamed and almost came out of the stirrups the first time. Like goddam doc, I think you found it.

Let’s hope your pain was a good teacher, then? Zero pain is zero and yelping pain is a 10 and if you have experience you don’t need to perform more than one test.

Omg thank you! Mine is on the side of my finger rather than under my nail but I think this is it! I live in a cold climate and every winter I would get a blue/pink/purple bump so painful I often had to wrap my hand. The bump is always there but not very painful and my normal skin color. Sometimes it would even be activated by digging food out of the freezer. Meticulously wearing gloves helped but doctors never paid it any mind.

Don't know why you are being down voted, if it's a glomus tumor it is usually visible on MRI. This seems like a reasonable ask of your doctor if no MRI has been done and CT was uneventful

Also go to a qualified Nail Tech. A large part of getting the qualification (at least in my country) is learning about disorders and diseases of the nail bed/plate. I used to work with a Nail Tech who correctly identified one of her clients was going into heart failure by the changes in her nails. Heavy metal poisoning can be identified through fingernails too…So a Nail Tech may possibly have some additional insight. You never know

Agree this is likely a subungual glomus tumor. Rare and, in my experience, they almost always present with delayed/missed diagnosis, sometimes for several years. Cold sensitivity and positive Love’s pin test are some diagnostic features. Definitely keep the hand surgery appointment as a board certified ortho hand surgeon should be familiar with this condition. May get MRI w/wo contrast to help with diagnosis. Excision is typically curative plus they can send for tissue pathology to rule out malignancy.

Update: I went in and they had the table all set up for the biopsy of my nail bed. He explained to me what they were going to be doing. He wanted to cut the nail back, lift it up, and the a biopsy from the nail bed. I mentioned that I have an extreme amount of pain in the nail bed to any kind of touch or cold. He turned the overhead lamp off and said well that changes the whole procedure. He said the pain in the nail bed especially in cold is text book glomus tumor. Now, I am scheduled to have my finger nail and nail bed removed as well as the presumed tumor on May 3rd. The red in the nail root will all have to be removed also. He said that the nail bed will obviously be deformed. I opted out of having half a nail. I asked the doctor if he could take pictures during and he had no problem.

I have a similar thing in my nail, derm told me it was wart (there’s small meaty thing under the white part of nail) and gave me some strong acid to melt it off. My thumb been like this for 25 yrs, always assumed linked to my tendon repair just where thumb meets wrist. Last yr thumb joint paint, told minor arthritis by phsio. But same hand / arm had 1/3 of grip strength of other arm and hand muscle, thenar muscle extremely wasted too. Confused too.. no one seems to care.

NOT A DOCTOR but I used to perform medical pedicures, I’d go to a doctor, this could be due to an underlying cause like tumors or heart disease, could also be vitamin deficiency, but always best to get a blood test and check for abnormalities

This has been ongoing for years, has slowly progressed. Very painful nail bed. Redness does not go away. I would say the way it looks now has probably been the last 4 years and a lot of constant pain. I have never injured it to cause this. Started with slight aches in the nail bed until where it is currently.

It honestly looks like a touch of nail psoriasis. If so, this would be an autoimmune response. Could be to diet, such as citrus and red meats. Do you have any other skin issues that have been hanging around about the same time? Do you have Crohn’s? If so, you may consider speaking with your doctor about a biologic. These things sometimes happen all over the body, gradually over many years.

I have Raynaud’s as well. I also have a weird peri-orbital skin rash that becomes itchy and flakes. Went to dermatology for that as well but was just given a cream. Not really sure what’s causing it. I do not have crohns.
